On line 47, the value for the givens field is "Mary V.E.", the suggested correction is "Mary G.E.". Submitted by Fran Irwin.
This correction was evaluated by Mark Gallop: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
