On line 6, the value for the givens field is "Catharin", the suggested correction is "Catherin". Submitted by Diane Cardinal.
This correction was evaluated by Brad Babcock: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
