On line 4, the value for the surname field is "Walls?", the suggested correction is "Watts". The following comment was attached: looking closely I think the pen was almost out of ink when he gave the line across the top and he overwrote the bottom without redoing the crossing of his t's?
This correction was evaluated by Anon: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
