On line 29, the value for the givens field is "Samuel H.", the suggested correction is "Samuel S". Submitted by Ghislaine Joiner.
This correction was evaluated by Mark Gallop: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
