On line 12, the value for the givens field is "Thelomènce", the suggested correction is "Philomène". Submitted by Alain Lévesque.
This correction was evaluated by A Little Bit At A Time!: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
