On line 17, the value for the surname field is "Perrott", the suggested correction is "Perrett". Submitted by Gord Haddow.
This correction was evaluated by Ron Awde: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
