On line 9, the value for the givens field is "Marie-Anne", the suggested correction is "Marie-Anna". Submitted by Serge Goudreau.
This correction was evaluated by Mark Gallop: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
