On line 39, the value for the givens field is "Gerl", the suggested correction is "Gord". Submitted by Geoffrey Wyght. The following comment was attached: I bet this is a sloppy "Gord".
This correction was evaluated by Lorna Madill: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
