I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
This comment was added: See Line 23 where he writes 'Guy' and you see his 'G' is very very clear. This could be Jarret but on line 22 he writes 'Jack' and the 'J' is very very clear.
