On line 17, the value for the givens field is "Maryann", the suggested correction is "Mary ann". Submitted by Margaret Cox.
This correction was evaluated by L.M. Bradley: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
