On line 8, the value for the givens field is "Geogeana", the suggested correction is "Georgeana". Submitted by Leslie Nutbrown.
This correction was evaluated by Mark Gallop: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
