On line 5, the value for the givens field is "Wilfred", the suggested correction is "Wilford". Submitted by Cheryl Herron.
This correction was evaluated by Mark Gallop: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
