On line 36, the value for the surname field is "Vianomess(?)", the suggested correction is "V?anomen(?)". Submitted by Norma Maus.
This correction was evaluated by Barbara Rose: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
