On line 3, the value for the givens field is "Willsem J.", the suggested correction is "William J.".
This correction was evaluated by Jean Lampi: I have checked the image of the original form and am very sure that the suggested correction does not reflect the original. This comment was added: This one is not clear. Rather than William I thought it might be Wilson but mispelt. I think I'll leave it for now since I still cannot see William in this name.
