On line 24, the value for the givens field is "Tina", the suggested correction is "Lena". Submitted by Tracey Sheady.
This correction was evaluated by Jason Porteous: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct. This comment was added: Looks more like a T as in Tilley below more so than L like Louis below.
