On line 20, the value for the givens field is "Minne", the suggested correction is "Minnie". Submitted by Shelley. The following comment was attached: under magnification it looks like the last 2 letters are very close together, with what looks like a dot for an "i"
This correction was evaluated by Donald Giles: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
