On line 3, the value for the givens field is "Manie?", the suggested correction is "Marise". Submitted by Barbara Broden.
This correction was evaluated by Michel Dubois: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
