On line 32, the value for the age field is "2", the suggested correction is "1". Submitted anonymously.
This correction was evaluated by Peggy Boyd: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct. This comment was added: While there appears to be a "1" written also, the "2" would appear to be more dominant
